The
Screening Report is a unique patient "periodontal report card".
It comes as a "tear-off" sheet, in prescription pad size and format and allows
the patient to leave with information about his or her own periodontal
condition. |

| The Screening
Report should be marked with the patient's periodontal status, either Health,
Gingivitis or Periodontitis, including which sextants have gingivitis, plus any
recommendations for treatment. This report should be reviewed
with and then given to the patient. This "take-home"
screening report will give the patient an opportunity to think about any
problems or recommendations indicated on it by reviewing the pictures of
probing and the educational information on the backside. The report will also
enable the patient to share the information with a spouse or family member in
an unhurried and non-threatening environment. |
